Your Sadio patient portal
Horizon is preparing to open in March 2027. Once you have received portal access, use Sadio to review your information, complete requested forms, and manage your appointments.
Getting started
When portal access is available to you, use the email address associated with your patient account. Check your inbox and spam folder for an invitation from Sadio.
If you already have an account, use the “Forgot Password” option on the Sadio website if you need to reset access. Further practice contact details will be shared closer to opening.
